Your First Appointment

Please allow at least an hour for your first appointment.

When you arrive, you will be asked to show proof of insurance. We also ask you to sign a consent form. We listen carefully to your story and take a thorough patient history.

A physical examination is also performed which may include X-rays. We will then design a multi-phased regimen for your treatment and begin your treatment on the first day.

To the best of your knowledge, either fill the forms out in Acrobat and print them or print them out and fill them in by hand. Having it completed will save time on your first visit.
